Use this form to enter the patient's Hebrew name as the title of an article, and also as the content of the article. At the bottom of the form, enter a date on which to stop publishing the name.
The name will appear on our website and in our newsletters so that prayers will be said for the patient's complete and speedy recovery.
Construct the Hebrew name with reference to the patient's mother. For example: Yitzchak ben Sarah, not Yitzchak ben Avraham.
If any part of the Hebrew name is unknown, use Sarah or Avraham for the unknown parts of a Jew's name. Use Chava and Noach as Hebrew names for non-Jews.
Don't let any Refuah List item outlive the need for it, as this might cause a b'rachah said in vain. To help prevent this, always assign a 'Finish Publishing' date. If the duration needs to be changed, you can come back and edit the date.
(As an additional precaution, the Refuah List shows visitors when each item was last modified, so worshippers can see if an item is stale.)
